ELIGIBILITY
• Birth to age 3 years
• Residence in San Diego or Imperial County
• No financial qualifications
• Developmental delay in one or more of the following five areas: Cognitive, Adaptive, Physical, Social/Emotional, Communication
• Younger than 24 months:
33% delay in one developmental area
• 24 months and older:
50% delay in one developmental area or
33% delay in 2 or more developmental areas.
OR
• Established Risk for developmental disability: Conditions known to cause delays in development.
(e.g. Down syndrome, Prader-Willi, Spina Bifida). Need not be demonstrating delays at time of referral.
Prevention Program- Birth to Age 3
ELIGIBILITY
• Birth to age 3 years
• Residence in San Diego or Imperial County
• No financial qualifications
• At risk for developmental disability (two or more factors):
◦ Less than 32 weeks gestation and/or birth weight less than 1500 grams
◦ Assisted ventilation for 48 hours or longer during first 28 days of life
◦ Small for Gestational Age
◦ Asphyxia neonatorum
◦ Severe and persistent metabolic seizures during the first 3 years of life
◦ Neonatal seizures or nonfebrile seizures during the first 3 years of life
◦ CNS lesion or abnormality
◦ CNS Infection
◦ Biomedical insult (injury, accident or illness) which may affect developmental outcome
◦ Multiple congenital anomalies
◦ Prenatal exposure to teratogens
◦ Prenatal substance exposure, positive tox screen or withdrawal
◦ Clinically significant failure to thrive
◦ Persistent hypertonia or hypotonia
OR
• Parent of the infant/toddler is a person with a developmental disability
OR
• Age 24 months or older with 33-49% delay in one of the following developmental areas: ◦Cognitive ◦Adaptive ◦Physical ◦Social/Emotional ◦Communication
ELIGIBILITY
• Age 3 or older (includes adults)
• Residence in San Diego or Imperial County
• No financial qualifications
• Developmental Disability: Mental Retardation, Cerebral Palsy, Epilepsy, Autism, Other conditions similar to mental retardation
AND
• Originated prior to age 18 years
AND
• Is likely to continue indefinitely
AND
• Constitutes a substantial disability in 3 or more of the following areas: Communication, Economic Self-Sufficiency, Learning, Self Care, Self-Direction, Mobility, Capacity for Independent Living
In order to process intake, SDRC must be contacted by the parent or legal guardian of a minor, the conservator, or the unconserved adult.
