Prevention Program

 

The Prevention Program services children ages 0 to 3 years old who are at a high risk for developmental delay or disability.  There are no financial qualifications for the Prevention Program and services are provided at no cost to families.

 

There are three eligibility criteria for the Prevention Program:
--the infant or toddler has two or more of thirteen defined high risk criteria.  Some examples include low birth weight, assisted ventilation during the first 28 days of life, multiple congenital anomalies.
--the parent of the child is a person with a developmental disability.
--the child is between the ages of 24 to 35 months and San Diego Regional Center determines through evaluation that the child has a 33% to 49% delay in one of the following areas: cognitive, physical, communication, self-help/adaptive or social/emotional.

 

The purpose and primary focus for the SDRC Prevention Program is to provide intake services, assessment, case management and referral to generic or community resources. 

 

Upon determination of eligibility for the Prevention Program and with the collaboration of the child’s parents and family, a Prevention Program Plan (PPP) will be developed and will include identification of resources and referrals and plans for monitoring the child’s development.
