KARAPAT-DAPAT:

Pagkaantala ng pag-unlad sa isa o higit pa sa mga sumusunod na limang lugar: Cognitive; Komunikasyon; Panlipunan/Emosyonal; Pisikal; Adaptive.

25% na pagkaantala sa kahit isang developmental area.

Walang kinakailangang mga kwalipikasyon sa pananalapi.

Itinatag na Panganib para sa kapansanan sa pag-unlad: mga kondisyong kilala na nagdudulot ng mga pagkaantala sa pag-unlad (hal. Down syndrome, Prader-Willi, Spina Bifida). Hindi kailangang magpakita ng pagkaantala ang bata sa oras ng referral.

Nasa panganib para sa kapansanan sa pag-unlad (dalawa o higit pang mga kadahilanan):

Wala pang 32 linggong pagbubuntis at/o timbang ng kapanganakan na mas mababa sa 1500 gramo.
-
Tinulungan ang bentilasyon sa loob ng 48 oras o mas matagal sa unang 28 araw ng buhay.
-
Maliit para sa gestational age.
-
Asphyxia neonatorum.
-
Matindi at patuloy na metabolic seizure sa unang 3 taon ng buhay.
-
Mga neonatal seizure o non-febrile seizure sa unang 3 taon ng buhay.

CNS lesyon o abnormalidad.
-
Impeksyon sa CNS.
-
Biomedical insult (pinsala, aksidente o sakit) na maaaring makaapekto sa resulta ng pag-unlad.

Maramihang congenital anomalya.

Prenatal exposure sa teratogens.

Prenatal substance exposure, positive tox screen o withdrawal.

Clinically makabuluhang kabiguan upang umunlad.

Ang patuloy na hypertonia o hypotonia.

MGA SERBISYO:
Pagsusuri sa limang bahagi ng pag-unlad upang matukoy ang pagiging karapat-dapat. Pagbuo ng isang Individualized Family Service Plan (IFSP), kabilang ang edad 3 transition planning. Koordinasyon ng mga kinakailangang serbisyo ng Maagang Pamamagitan batay sa mga pangangailangan ng bata. Maaaring kabilang sa mga serbisyo ang: Assistive Technology; Mga Serbisyo sa Audiology; Pagsasanay sa Pamilya, Pagpapayo, Pagbisita sa Bahay; Serbisyong pangkalusugan; Mga Serbisyong Medikal (mga pagsusuri para sa mga serbisyo ng maagang interbensyon); Mga Serbisyo sa Pag-aalaga; Mga Serbisyo sa Nutrisyon; Occupational Therapy; Pisikal na therapy; Mga Serbisyong Pangkaisipan; Koordinasyon ng Serbisyo; Sign Language/Cued Language Services; Mga Serbisyo sa Social Work; Espesyal na Pagtuturo; Patolohiya sa Pagsasalita at Wika; Transportasyon at Mga Kaugnay na Gastos; Mga Serbisyo sa Paningin.

ELIGIBILITY

Resident of San Diego or Imperial County.

No financial qualifications necessary.

Developmental disability:
-
Intellectual Disability – When a person has certain limitations in mental function and adaptive skills. The limitations will cause a child to learn and develop more slowly than a typical child.

Cerebral Palsy – A disorder that affects body movement and muscle coordination. It can be caused either when the brain does not develop properly during pregnancy or if there is damage to the brain before, during, or after birth.

Epilepsy – A neurological disorder that is characterized by recurrent seizures. Seizures may involve partial or complete loss of consciousness, uncontrolled body movements, excessive sleepiness, and loss of memory.

Autism – is a disorder that affects an individual’s ability to communicate, understand language, play, and relate to others.
-
Other conditions found to be closely related to an Intellectual Disability or to require treatment similar to that required for individuals with an Intellectual Disability.

AND

Originated prior to age 18 years.
AND
-
Is likely to continue indefinitely.
AND
-
Constitutes a substantial disability in three or more of the following areas: communication; learning; self-direction; capacity for independent living; economic self-sufficiency; self care; mobility.
SERVICES:
Evaluation during intake process if needed to determine regional center eligibility. If eligible for regional center, assessment to assist with program planning. Development of the Individual Program Plan (IPP). Case Management/Service Coordination. Coordination of developmental disability services which may include: Residential Services; Transportation; Nursing Services; Behavior Intervention; Medical Services; Work Program; Psychological Services; Respite; Physical Therapy.
PROVISIONAL ELIGIBILITY
A child who is three or four years of age who is otherwise not eligible for regional center services as a result of a developmental disability shall be provisionally eligible for regional center services if a child has a disability that is not solely physical in nature and has significant functional limitations in at least two of the following areas of major life activity as determined by a regional center and as appropriate to the age of the child:
